Yizkor Book Form

Our Book of Remembrance/Yizkor Book is an important tradition at The Emanuel Synagogue, and contains the names of those whom you wish to remember during Yizkor. 

The Book includes two sections:

Section #1: Book of Life:. For a one time charge of $360 per name, you create an Eternal Memorial, as your loved one's name will be listed in the Book of Life section for current and future years.

If you already have names included in the Book of Life,  CLICK HERE. to review your most recent submission. If there are no changes, please select YES  on the form below, confirming the same names are to be printed with no additions or corrections. Then scroll to the bottom and click 'submit'.

Section #2: We Remember Them: The Annual Memorial section charged each year the name is submitted for inclusion, along with the name of who they will be remembered by.  Fee is based on the number of names submitted, names need to be re-submitted every year

• 1 to 4 names - $25 per name
• 5 to 7 names - $125 total
• 8 to 10 names - $150 total
• 11 to 13 names - $175 total
• 14 to 20 names - $200 total

Name submission deadline August 26, 2024, in order for the book to be printed in time for the High Holy Days.
